My aunt is Patricia Hammer. I had my first visit with her today. You see, I had put going to see her for way too long. The reason is I spent 4 years visiting my grandparents in the nursing home and lets just say it was not the same kind of experience!! I can not say enough good things about my experience today!!!!! The place is as clean and smelled like it. The staff was extremely helpful and I felt like I was around friends and family!!! I am just so grateful

This review is mostly based on the staff that support the person I visit. They're extremely personable and tend to go well beyond their assigned duties to ensure their client feels cared for. This is all in light of us knowing they have a heavier workload than normal and are surely dealing with great stress. Very admirable.

I gave 5 stars because the workers were very attentive, the Tech that was looking after my aunt, she was so nice and you could tell she is caring young woman, she made a point to come in and introduce herself. I forgot her name, but she's a keeper, we had some questions, she got right on top of it. The young lady that brought lunch, she helped them cut up their food. It was comforting to see that they all cared about the patients.

Felt my family member is being well cared for. Saw his physical therapist and his doctor during my visit. Both were thorough and explained the situation and treatment very well. The facility was very clean, all of the staff were friendly and welcoming.

Today was day one of my wife's stay at Creekside for rehabilitation. Upon entering the facility I see a very attractive and inviting place. The people are very nice and the facility is clean and inviting. When I got to her room it also was very clean and bright and cheery. She is pleased with everything and I asked her about the food. She said it was well-prepared and even though she is on a renal diet the food choices she also liked. At this point we have not seen anyting negative. Kudos 2 Creekside.

I was recently in Creekside for rehabilitation following seven weeks of hospitalization. I had a lovely view of the courtyard and went outside to sit there many days. My first contact was the patient concierge. She was compassionate, caring, and ready to do whatever I needed to enhance my experience there. I received wonderful care from the nursing and tech staff. They were the best—kind and willing to help. I received excellent therapy, both physical and occupational. The team worked with me to restore my strength and did a great job of preparing me to go home. Overall a really good experience!

My mother was here recently after suffering a fall in her home. We experienced nothing but the best of care and very warm and caring nurses and staff, and a clean and well run facility. All facilities of this type can end up a little short staffed occasionally, and all employees at every job can have a bad day every so often, that's just the nature of things everywhere. But you can tell that the leadership here and the staff genuinely care and do their best. My mother has been in several such facilities over the years, and this was the best one of them all - by far.

About one year ago my sister moved to Creekside Health and Rehabilitation from an assisted living. She has THRIVED physically and emotionally since moving. Her health and mood have improved tremendously. It's amazing to see. There's always something to do to keep her busy instead of laying in bed all day. I also appreciate the calls we receive to update us on her status as well as the "goings on" at Creekside. I would not hesitate to recommend this facility.

This is the first Nursing Home Facility that I have entered on a daily basis that has not had the "Nursing Home" smell, and I have been in many! That said, the staff, from the cleaning crew to the Administration, always greets me as I pass in the hall. My Father has been here for several months now and is way in the back so I pass a lot of staff getting to his room. I have yet felt like I have to ask a lot of questions because I am informed before I even have to ask. Let me tell you, this is not the "Norm" for most facilities. Anytime I need assistance it is given happily, not once have I felt like I was bothering anyone. The Nurses are knowledgeable and caring as is the rest of the staff. When the Admissions Administrator called to tell me that they had a place for my dad it was as if a huge burden had been lifted and every since I have known this was the right choice for us. Can't say enough good things about this place and the people there. Thankful I did not go by the reviews I read and instead went and checked it out for myself. I was informed the management changed about 6 months ago, it must have been a great change!

My father was at creek side for rehab after being in the hospital and they helped him in a variety of ways. They healed an issue on his foot he had been battling for years, they clearly communicated a care plan and called me frequently to give me updates. He is a picky eater but gained weight and loved the food while he was there. He can be difficult at times but they clearly showed him caring and patience while he was there.

What a blessing Creekside was to my sister and her family! She lived at Creekside for the last three months of her life. We could not have asked for a more caring and accommodating “home” for my sister. I am usually inclined to speak up when I see things that I believe are wrong or a patient is not being treated in the manner which they deserve. I can honestly say that I never saw treatment of my sister that disturbed me in any way. On the contrary, she was treated with kindness, gentleness, compassion and caring. But most importantly to me, she was always treated with dignity and respect. The host at the front door, the housekeeping personnel, the nursing aides (particularly Vivian), the nurses and the administrator were all blessings to us. I will never forget the peace of mind that I had each time I left my sister in their care. I wholeheartedly recommend Creekside.

My husband had a recent stay at Creekside Center due to a very serious infection in a surgical wound. The cancer diagnosis and surgery were bad enough but the infected wound came closer to taking his life. The wound care that Beth and Nikki provided was spot on. Even better than the care they compassionately provided was the training I received to continue his care at home. From the time he got there until he came home everyone, no matter who they were or what they did were so professional and encouraging during a very trying time. AND I am happy to report his wound has completely healed. We are thankful every single day.

My grandmother, Oleda Franklin had been very sick leading up to her stay at Creekside. First it was Covid, then when she got to the hospital at skyline, she was transferred to Murfreesboro in a nursing home that was horrible. When she was transferred back home to Madison at Creekside, things were much better. They seem to genuinely care and we had great response from them as well. During this time with Covid, it was difficult because we weren’t allowed to go in and see her at first. We especially were really impressed with our concierge, jean marie. She took what could have been a horrible situation and connected us with my grandmother. She also had this way about her that put my grandmother at ease. There are too few people in this world with the kind of heart that she showed. My grandmother has since passed, but our stay at Creekside was very good and would recommend to someone in the future.

Creekside and the staff are absolutely amazing! The care they provide for my father is second to none. Also, they do an amazing job at communicating with me and informing me on any changes or needs my father may have. If you’re looking for a place where you’re loved ones can be cared for like family and well taken care of... Creekside is the place you should go.

Creekside center!!!!!!!! Its a excellent place for my mom. They really care for their patients. The place is a very clean and genuine environment. I have never once felt that my mom wasn't in a safe place. They push that they give the patients in therapy is I couldn't be thankful enough. And most of all I've never once seen a patient that wasn't treated like family. I have alot of love and very thankful for Creekside center. Wouldn't have my mom no other place. Thank you Creekside

I have been here before and was glad to come back. The therapists here (both OT & PT) are very good, pushed me gently to my limits and helped me recover. I found responses to the nurse-call button were slow and I hate that someone must come to see what's needed and then go and get it. Two trips instead of one. Wasteful. Sometimes my food was delivered cold, and once my supper was not delivered at all. An alternate meal was brought later. Otherwise, it was another good stay.

When I first came here, I could not walk or be independent. Within a week, with the help of therapy, I have progressed considerably!!! My mother was at Creekside about a month in March-April during COVID precautions and “lockdown”. And I was in California unable to visit here. The physical therapy staff kept me informed of what they were doing and her progress. Most outstanding was the nurse “concierge” that connected me with my mother thru FaceTime video calls. Thus was actually good for my mother and invaluable for me to evaluate how she was doing. I am glad we made decision to put here at Creekside for her rehab.

My name is Sharon Cookie Morgan an I was in Creekside for Rehap from 1/6/20 until 1/16/20. During my stay the nursing staff, respiratory therapist, techs, dietary and physical therapists were all pleasant, attentive and accommodating. I would thank the staff for making my recovery easy.

Creekside Center For Rehabilitation And Healing Is A Warm, Clean, Safe, Loving, Facility that I Recommend highly with a Five Star * Rating . I can say this Personally Because I Was A Resident There. The Staff And Administration Treated Me Warmly And With Open Arms. There Were No Lying In Bed Unless You Were ILL. The Physical Therapy Department, The Activity Department we’re on their job . There Was Always Something To Do. Healing Is In Creekside ‘s Name Because Not Only Do They Work On Healing Your Body, But Your Mind And Your Soul. The Doctors And Nursing Staff Was So Caring, With Excellent Bed Side Manner. The Administration Staff Was Very Knowledgeable. I Was Very Happy And Found No Complaints.

Upon my arrival at Creekside from a horrible vehicle crash I was somewhat set back ( a rehab facility inside an Elderly Care Center ) not what I expected. It didn’t take long before I realized what a mistake I had made prejudging Creekside. The scales began tilting in there favor immediately, and has continued every day. I would rate them 5 stars in every area of my care especially the rehab staff. Thank you Creekside for getting me to a point that I can now go home and take care of myself !!!

Checked my wife out 2 days ago. She was there for 15 days because of heart surgery rehab. We were very impressed with the facility and the people working there. The care and attention she received from everyone from the nurses, aids, therapists, and cleaning people was beyond expectations. It is an older facility but kept to very high and clean standards. Even the food was good and very consistent.
