Overall 5 stars ⭐️ Dr Bollenbaugh made my experience very convenient and easy. I communicated directly with him not through a secretary which made the process much easier. I recommend using him because the large corporate companies have their own company policies that make things a lot harder to deal with. I personally went to him because another corporate DOT physical company wouldn’t take a medication approval form from my primary care physician at the VA because of their company policy. Very informative. 30mins was all it took. Recommend this doctor. Will return in 2 years for my next. Conveniently located at the TA truck stop downtown Nashville across Nissan Stadium it’s perfect for in and out service he gets you back on the road in no time.

Dr. Bollenbaugh is great. He has an awesome set up to get you in and out for the Med Card Physical. I made an appointment with him, he communicated well with me during the process. He texted to let me know he was finishing up with the previous client and the process was very good. He takes care of getting the right paperwork in your hand to file with the Department of Safety. I could not have asked for a better situation to renew my CDL's. Give him a call and set up an appointment when it's time to get your Med Card Physical. You will be glad you did.

Dr. Bollenbaugh is by far the best DOT Medical Examiner I have ever been too. This is the second time I have been to his office. I don’t currently drive a truck, but keep my license just in case I start driving again. Finding a good down to earth medical examiner who cares about what he does is tough and I am glad I found the one I will be returning too. He was able to see me next day, at a good time and the exam was a lot quicker than a traditional doctors office that takes forever for the doctor to come in. I’m not very fond of doctors, but feel fine here. Thanks Dr. Bollenbaugh, I will be returning again when my Med Cert expires again!

Flexible and friendly. Understands the rules very deeply. Willing to work with you to get your DOT physical for trucking jobs and delivery jobs, even if you have (properly and fully treated, with letters from your doctors) mental health issues. (With some exceptions, of course). Concentra won’t do that, usually. Trustworthy!
